Healthy Care Solutions offers a Sliding Fee Scale opportunity to all income eligible, uninsured, or under-insured patients. This program allows qualifying patients to receive our clinical mental health services that do not discriminate on the basis of an individual’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.
Eligibility for the Sliding Fee Scale will be established by determining the household size and the annual household income. This information must be updated annually. A completed application requires valid proof of income, household size, and completion or a payment agreement upon acceptance into the program.
This application process can take up to 10 business days and is expected to be complete prior to providing services. The scaled rates are dependent on the current Federal Poverty guidelines and the percentage from a scale of 0-200%. No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income.
